This form is to be completed by individuals requesting a customized wallpaper/digital image. The completed form should be submitted to and approved by the Athletics Department. IMPORTANT: Generally, per NCAA rules and regulations, it is impermissible to provide customized images to prospective student-athletes (some exceptions apply).

13.02.13 Prospective Student-Athlete – Generally, A prospective student-athlete is a student who has started classes for the ninth grade. In addition, a student who has not started classes for the ninth grade becomes a prospective student-athlete if the institution provides such an individual (or the individual’s family members or friends) any financial assistance or other benefits that the institution does not provide to prospective students generally…
13.4.1 Recruiting Materials and Electronic Correspondence – An institution shall not provide recruiting materials, including general correspondence related to athletics, or send electronic correspondence to an individual (or his or her family members) until June 15 at the conclusion of his or her sophomore year in high school.
13.10.2.1 Comments Before Commitment. – Before the signing of a prospective student-athlete to a National Letter of Intent or an institution's written offer of admission and/or financial aid or before the institution receives his or her financial deposit in response to its offer of admission, a member institution may comment publicly only to the extent of confirming its recruitment of the prospective student-athlete. The institution may not comment generally about the prospective student-athlete's ability or the contribution that the prospective student-athlete might make to the institution's team; further, the institution is precluded from commenting in any manner as to the likelihood of the prospective student-athlete committing to or signing with that institution.
